Travel MDS Nurse (RN or LVN) – Hybrid Remote | Central CA & SF Bay Area Are you an experienced MDS Coordinator looking for flexibility, variety, and a chance to make a real impact?
Evergreen Healthcare Group is hiring a Travel MDS Nurse (RN or LVN) to support skilled nursing facilities across Central California and the San Francisco Bay Area in a
50% remote / 50% on-site hybrid role. What You’ll Do: - Support clinical teams with timely and accurate MDS assessments
- Coordinate ARDs and IDT participation across multiple facilities
- Conduct on-site visits for training, audits, and assessment support
- Utilize EMR systems to complete MDS documentation and ensure regulatory compliance
- Promote quality outcomes through collaboration with facility leadership
- Step in to support urgent MDS coverage needs at facilities experiencing staffing gaps, including temporarily filling in during transitions until a permanent replacement is hired.
Requirements: - Current RN or LVN license in California (unencumbered)
- 4+ years as an MDS Coordinator in a skilled nursing facility
- Strong understanding of the RAI process and care planning
- Comfortable with travel across Central CA & SF Bay Area
- Proficient in EMR systems (PointClickCare preferred)
Preferred: - RAC-CT Certification
- Medicare utilization review experience
- Strong documentation and analytical skills
