Key Responsibilities:

1. Plan, organize, implement, evaluate, and direct the overall operation of the Social Services Department.
2. Participate in discharge planning, development, and implementation of social care plans and resident assessments.
3. Interview residents and families to obtain social history and involve them in planning social service programs.
4. Refer residents and families to appropriate social service agencies for additional support.
5. Provide information to residents/families on Medicare/Medicaid, and other financial assistance programs.
6. Collaborate with staff, medical professionals, and other departments to address the social and emotional needs of residents.
7. Ensure all social services activities comply with federal, state, and local regulations.
8. Maintain accurate and informative charted progress notes.

Qualifications:

1. Bachelor’s degree in social work or a related human services field (e.g., sociology, rehabilitation counseling, psychology).
2. Minimum of 3 years of supervised social work experience in a healthcare setting.
3. Experience working in a Skilled Nursing Home Facility.
4. Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) preferred.
