Job Title: Registered Dietitian / Nutritionist - Diabetes Education Program
Location: Shiprock, NM.
Start Date: April 2025
Shift: 8am - 4:30pm 40hours week
Job Description:
We are looking for qualified Nutritionists for the Diabetes Education Program. The Public Health Nutritionists will be responsible for planning, implementing, and evaluating nutrition education and counseling services for patients with diabetes. Will work collaboratively with other healthcare professionals to develop individualized nutrition care plans.
Requirements:
- Bachelor's or Master's degree in nutrition or a related field.
- Registered Dietitian (RD) or Nutritionist with the Commission on Dietetic Registration (CDR).
- Experience in providing nutrition education and counseling services to patients with diabetes.
Responsibilities:
- Conduct nutrition assessments and develop individualized nutrition care plans for patients with diabetes.
- Collaborate with healthcare professionals, including physicians, nurses, and dietitians, to provide comprehensive care for patients with diabetes.
- Monitor and evaluate patient progress and adjust nutrition care plans as needed.
- Document patient care activities and maintain accurate patient records in accordance with established guidelines and regulations.
- Participate in team meetings and other activities to improve patient care and outcomes.
