Job Summary: .
Coordinates outpatient programs that provide follow-up to infants in the High-Risk and NICU follow-up clinics and the Pediatric Health Center (s), and Pediatric Services. Involves the identification of follow-up patients, meetings with the family to review program, appointment setting, weekly clinic set up and management, and coordination of the typing and mailing or evaluation summary letters to private pediatricians. Coordinates care for infants/ children and prepares reports that are performed as baseline and/or for diagnostic purposes. Organize and administer individualized age-specific professional care to patients with an assigned unit of the hospital in support of medical care. Practices in accordance with the State Nurse Practice Act, American Nurses Association Standards of Practice, Code of Ethics for Nurses, Department of Health Regulations, JCAHO requirements and other regulatory agencies. Participates in patient teaching and discharge planning. Participates in Unit Performance Improvement evaluation activities. Supports the philosophy, objectives and the mission of Cooperman Barnabas Medical Center. Participates in patient teaching and discharge planning.

Essential Functions:
Organize and administers individualized age-specific professional care to patients with an assigned unit of the hospital in support of medical care. Work closely with neonatologists, pediatricians, and other healthcare professionals to coordinate patient care and discharge planning.
Engage with families, providing education and support during complex decision-making processes regarding their infant s care. Assists with revenue cycle, billing, coding and follow-up for NICU, High-Risk infant follow-up, Pediatric Health Center, Pediatric Services.
